How do I get started?
To participate in Diabetes self management training you will need a referral from your physician. Please contact your Primary Care Provider (PCP) or Endocrinologist about enrolling in our program. If you do not have a Primary Care Provider, please call our toll-free Physician referral line at 497-DOCS or 1-877-497-DOCS (3627) or search online.


St. Elizabeth Diabetes Center

Though diabetes is a serious health condition, people with diabetes can lead healthy lives. As a person with diabetes, you need to learn all you can about self-care and diabetes control.

The St. Elizabeth Diabetes Center is a comprehensive outpatient self-management education and training program for people of all ages with diabetes. You can learn more about meal planning, exercise, medication and minimizing possible diabetes-related health problems.

These services are offered in a group setting or as individual instruction and provided by physician referral. You will work with your physician and the St. Elizabeth Diabetes Center staff in the management of your diabetes.
